外国人患者の方

受診の際は、以下の書類を確認させていただきます。

  1. マイナ保険証または資格確認書

  2. 本人確認書類（写真付きの身分証：パスポートまたは在留カード）

なお、マイナ保険証または資格確認書の提示がない場合、日本の公的医療保険に加入していない患者さんにつきましては、自費診療となります。 日本語に不安がある患者さんについては、自身で医療通訳を手配してからご来院ください。

We will need to check the following documents during your visit.

  1. My Number health insurance card or qualification confirmation document

  2. I dentification documents（photo ID：passport or residence card）

Please note that if you are not enrolled in Japanese public health insurance and do not present your My Number health insurance card or eligibility confirmation certificate, treatment will be at your own expense.
Patients who are unsure about their Japanese language skills should arrange for a medical interpreter before visiting the hospital.
